Analysis Request Forms

The Ministère provides analysis request forms that complies with the requirements of section 30 of the Regulation respecting the quality of drinking water–hereinafter, “the Regulation” for each type of required control, based on the specific characteristics of a given distribution system. Accredited laboratories can directly access these analysis request forms in the Ministère’s système informatique Suivi de l’eau potable (SEP), for the French version. These laboratories may use different forms as long as they include all of the elements of the Ministère forms in a similar presentation format.

Water distribution system responsibles are required to retain copies of all analysis request forms sent to their accredited laboratories for at least two years and make them available to the Ministère upon request (section 30 of the Regulation, last paragraph).


Distributed water analysis request forms

All drinking water samples collected at a distribution system must be accompanied by an analysis request form that corresponds to the type of required control, signed by the sample collector. A distinct analysis request form is available for each type of distributed water control.
  • Distributed water microbiology (section 11)
  • Inorganics (sections 14, 14.1, 15 and 21)
  • Organics and THM (sections 18 and 19)

Raw water analysis request forms

All raw water samples collected at a production facility must be accompanied by an analysis request form that corresponds to the type of required control, signed by the sample collector. A distinct analysis request form is available for each type of raw water control.
  • Raw groundwater microbiology (sections 13 and 21.1)
  • Raw surface water microbiology (sections 22.0.1 and 53.0.1)
  • Raw surface water inorganics (section 22.0.2)
